Announcing the Willow Creek Health Foundation

The Willow Creek Health Foundation is a nonprofit organization dedicated to improving access to compassionate, patient-centered primary care in our community.  Our mission is simple: to help remove financial barriers so more individuals and families can receive the care they need, when they need it.

Donations to the Foundation help support:
• Direct primary care memberships for individuals and families
• Preventive care, labs, and essential medications
• Health education and community wellness efforts

 

Every contribution helps ensure that cost is not a barrier to receiving quality primary care. Together, we can make healthcare more accessible.
